Job Summary: Toni’s House, a supportive recovery residence for individuals overcoming addiction and homelessness, is seeking a compassionate and dedicated Part-Time Case Manager to join our team. The Case Manager will be responsible for managing resident care from intake through their time living at Toni's House. This includes helping residents create personalized, holistic recovery plans and setting both short-term and long-term goals. The Case Manager will also track progress, provide accountability, and conduct evaluations at key milestones. When capacity allows, the Case Manager may also assist individuals referred by the Street Team or Helpline.
Key Responsibilities:
- Resident Intake and Assessment: Conduct thorough assessments of new residents to understand their needs, strengths, and challenges, and create a welcoming environment for residents entering Toni's House.
- Holistic Recovery Plan Development: Collaborate with residents to design individualized, holistic recovery plans that include mental, emotional, and physical health goals as well as education, employment, and housing objectives.
- Goal Setting and Progress Monitoring: Work with residents to establish both short-term and long-term goals. Track and evaluate their progress regularly, adjusting the recovery plan as needed.
- Accountability and Support: Serve as an accountability partner for residents by offering encouragement, guidance, and consistent support throughout their recovery journey.
- Client Referrals (As Capacity Allows): Take on clients referred by the Street Team or Helpline, developing recovery plans and offering case management for individuals from the broader public.
- Digital Case Management: Maintain and update client profiles in the case management platform. Ensure all documentation is accurate, up-to-date, and in compliance with organizational guidelines.
- Milestone Evaluation: Conduct regular evaluations at designated milestones in each resident’s recovery journey to ensure that progress is being made and to provide constructive feedback.
- Community Resource Coordination: Connect residents with additional resources, including healthcare services, job training, and educational opportunities, to support their long-term success and reintegration into the community.
Required Skills and Qualifications:
- Experience:
- Minimum 2 years of experience in case management, preferably in addiction recovery, mental health services, or a related area.
- Experience working with individuals facing homelessness, substance use disorders, or other social challenges.
- Digital Literacy: Proficiency with digital case management platforms, data entry, and client record management.
- Interpersonal Skills: Strong communication, empathy, and active listening skills. Ability to build trust and foster a positive, encouraging environment for clients.
- Problem-Solving and Organizational Skills: Ability to create tailored plans for each resident, organize resources, and troubleshoot challenges as they arise.
- Cultural Competency: Familiarity with and sensitivity to the unique needs of diverse populations, including BIPOC, LGBTQ+ individuals, and veterans.
- Availability: Must be available to work in person in Las Vegas and maintain a flexible part-time schedule to accommodate residents’ needs.
Preferred Qualifications:
- Education: Bachelor’s degree in Social Work, Psychology, Counseling, or a related field. Master’s degree is preferred.
- Certification in addiction counseling or recovery coaching.
- Training in other relevant areas such as Trauma-informed care practice, Motivational Inquiry, Crisis Intervention, Harm-reduction, CBT and cultural competencies.
- Bilingual in English and Spanish.
Compensation: Competitive hourly rate, commensurate with experience.
